Advanced Health Care provides transitional short-term Skilled Nursing & Rehabilitation following a hospital stay and before returning home.

All five locations in Nevada provide short-term skilled nursing care for individuals with nursing and rehabilitation needs.

These include but are not limited to, physical therapy (PT), occupational therapy (OT), speech-language pathology (SLP), post-surgery management, wound care, disease management, intravenous (IV) therapy, and debility.

 

Workers’ Comp injuries requiring Post -Acute In-Patient Care

 

    • Traumatic Brain Injury
    • Spine & Lower Extremity Injuries, (non-ambulatory),
    • multiple fractures, or no one at home to help with care.
    • Burn Care
    • Wound Care Management
    • IV Therapy
    • Infection risk in home environments, or other risk factors impacting recovery, i.e. Diabetic, etc.

All Nevada locations have 38 Private Rooms, a warm, comfortable & supportive environment & professional dedicated staff. All AHC's Nevada facilities offer private suites and have received a 5-star ***** rating from the Centers for Medicare & Medicaid Services (CMS) year over year. This rating reflects excellent staffing ratios,  above-average outcomes, and exceptional patient experience. The care provided is managed by physicians and supported by staff such as physician assistants (PAs) and nurse practitioners (NPs) to ensure daily care management. Specialty physicians on staff to help meet specific needs of patients.

 

Physical, Occupational and Speech Language Pathology Therapists ( PT/OT/SLP):  

Up to 3-hours per day of therapy, based upon individual needs & abilities. The goal of AHC’s aggressive therapy program is to help patients attain maximum potential and restore functional abilities and confidence necessary to return home safely & independently resulting in shorter patient stays and RTW in the shortest time possible.

All meals are served restaurant-style and get top reviews! Cardiac and diabetic diets are served and supervised by registered dietician.

 

AHC Case Managers assist Adjusters & NCMs to coordinate delivery of progress reports, obtain authorizations, and arrange transportation for patient.

Facilitate transportation for follow-up appointments with doctors. Case managers will coordinate discharge planning or assist NCM in arranging for DME, Prescriptions, and other services as deemed necessary. Case Managers will be involved as much is necessary to coordinate a smooth transition upon discharge to home. Advanced Health Care will arrange transportation from the hospital to facility and help coordinate transportation home when patient is discharged.
